Inositol benefits: what myo-inositol and the 40:1 blend really do, scored on human studies
Cycle day 45 again, and your doctor offered birth control or metformin. We scored 8 inositol claims on 15 studies and 665 owner posts. Two hold up: lower insulin and more regular cycles. But the claim owners post about most has almost no proof. It's #6.

The short answer
Yes for long cycles and insulin resistance. Not much else.
In 9 studies of women with PCOS, myo-inositol lowered fasting insulin 1. In 10 studies, periods came more often and ovulation was 2.3 times as likely 4. Take it with food and give it 3 months. The bottle we'd buy is Dosera's 40:1 myo + D-chiro capsule, $17.50 a bottle. One number it barely moves: your morning blood sugar.

Cycle day 45 again? Why women with PCOS look up inositol
“Cycle day 45 and no period, no ovulation.” One woman wrote that on a fertility app forum. In the same thread she added: “I'm longing for the days when I had a regular cycle and despised it.” Others write “My doctor gave me 2 options one was birth control the other he said was a pill used for Diabetes.”
If that sounds like you, you want a period that comes on its own, dark OPKs and quiet sugar cravings. You may have quit metformin over “nausea and poop issues,” or been told “my doctor has not heard of Inositol.” Keep the woman on cycle day 45 in mind. In the timeline, we show what women like her reported by month 3.

What to look for in an inositol bottle
- Myo-inositol and D-chiro-inositol together, at 40:1
- Both amounts printed in mg, per serving and per capsule
- At least 2,000 mg of myo-inositol a day
- No proprietary blend hiding the amounts
- Lab-tested by batch
- One price and a money-back guarantee you can use
What to skip
- Gummies with a few hundred mg and added sugar
- D-chiro-only capsules at high doses
- “PCOS blends” that list one total for ten ingredients
- Bottles that print “2,000 mg” but mean 4 capsules without saying so
- Anyone who calls it a replacement for your prescription

Which inositol benefits hold up? All 8 claims, scored
Each claim is scored on three things: how many human studies tested it, how big the change was, and whether the studies agree. The weights are 45, 35 and 20. The best-supported claim is at the top.

Score = each part out of 100 × its weight. The weights are printed in “How we scored” below.
Only two claims clear the bar: lower insulin and more regular cycles. Everything else rests on one small study, a dose far above any capsule bottle, or a review that calls the evidence uncertain.
If those two are why you are here, this is the 40:1 bottle we'd start with. The cards below show what each study found, starting with insulin.
Every claim, scored
Lowers insulin in insulin resistance
- Human studies · 45%82
- Size of the effect · 35%72
- Studies agree · 20%80
The best-tested claim. Fasting insulin came down across 9 studies in women with PCOS.
This is the claim inositol is built on. In a review of 9 randomized studies with 496 women with PCOS, myo-inositol lowered fasting insulin and a lab score for insulin resistance 1. The authors ran an extra check on the insulin result and found it held up.
Your morning blood sugar may barely move, and that is not a sign it failed. A 2023 review of 26 studies with 1,691 women found fasting blood sugar only about 3 points lower than with a placebo pill 3. Inositol works on insulin first.
It is not only for PCOS. One 12-month study tested 80 women past menopause with high blood sugar, blood pressure and blood fats. On 2 g twice a day plus a diet, all three improved more than on diet alone 7. By the end, 8 of 40 women on inositol no longer met the definition of metabolic syndrome, against 1 on placebo.
Why not a higher score? The review behind the 2023 international PCOS guideline looked at 30 studies with 2,230 women. It found benefits on some metabolic measures, but called the evidence overall “limited and inconclusive” 2. We agree with both halves of that.
Makes cycles more regular and helps ovulation
- Human studies · 45%80
- Size of the effect · 35%74
- Studies agree · 20%68
Strong for periods coming more often. Weaker and more mixed for ovulation.
This is the claim most buyers care about. A review of 10 randomized studies compared inositol with a placebo pill in women with PCOS 4. Women on inositol were 2.3 times as likely to ovulate. Their periods came more often, with a 6.8 times higher chance of more frequent cycles.
The 2023 review of 26 studies found the same direction 3. Women on inositol were 1.79 times as likely to have a regular cycle as women on a placebo. It did about as well as metformin on this outcome.
The 40:1 ratio every forum repeats comes from one study that tested 7 ratios on just 8 women each 6. The 40:1 group did best for ovulation, but 8 women is a very small group. It is still the mix the bottle we picked uses, because no better test exists.
The guideline review also found no clear proof that myo-inositol beats metformin for ovulation. It rated much of the evidence low certainty 2.
Lowers the risk of high blood sugar in pregnancy
- Human studies · 45%72
- Size of the effect · 35%66
- Studies agree · 20%55
Promising in studies. Not a reason to start anything in pregnancy without your OB.
We include this because searchers ask about it. We report it as a study result only. One study tested 220 pregnant women who had a parent with type 2 diabetes. On 2 g of myo-inositol twice a day, gestational diabetes fell from 15.3% to 6% 8. Women with PCOS were not part of that study.
A 2023 Cochrane review pooled 7 studies with 1,319 pregnant women 9. Myo-inositol roughly halved the risk of gestational diabetes. But the reviewers rated the evidence low to very low certainty. Six of the seven studies were done in Italy.
If you are pregnant, or might be, ask your OB before you start or stop inositol.
Clears acne and slows chin hair
- Human studies · 45%55
- Size of the effect · 35%48
- Studies agree · 20%50
Some help in small studies. Slow, and metformin did better on hair.
In a study of 50 women with PCOS, acne and unwanted hair both went down after 6 months of myo-inositol 10. Testosterone and insulin dropped after 3 months. But that study had no placebo group, so we cannot be sure how much was the supplement.
The 9-study review found only a slight drop in testosterone, too small to be sure it was real 1. The guideline review found metformin did better than inositol on hair in 2 small studies 2.
Owners match the studies. Some say “hair growing back slower on my face.” Others say “I still get chin hairs though.” Hair takes months to change, so judge this one at 6 months, not 6 weeks.
Weight
- Human studies · 45%60
- Size of the effect · 35%30
- Studies agree · 20%45
A small effect at best. Do not buy it for this.
The honest answer is small. The 26-study review found BMI 0.45 lower with inositol than with a placebo pill 3. For a woman 5 foot 5, that is about 2.7 pounds.
In the 12-month study of women past menopause, BMI and waist size did not change more than with diet alone 7. The guideline review rated the evidence on BMI as very uncertain 2.
Improves egg quality and the chance of pregnancy
- Human studies · 45%50
- Size of the effect · 35%40
- Studies agree · 20%35
Uncertain. The studies are small and mostly in IVF clinics.
This is the claim owners post about most, and it has the least proof. A Cochrane review looked at 13 studies with 1,472 women with PCOS, most of them before IVF 11. The reviewers could not say whether inositol raises live births. The evidence was rated low to very low quality.
The 10-study review on ovulation found only one study that reported pregnancy against a placebo, and none that reported live births 4. Inositol may help you ovulate. That is not the same as proof it helps you get pregnant.
Owners do post dates, like “I got pregnant 41 days after starting Inositol!!” Those stories are real, but they are not proof it will work for everyone.
Eases anxiety and low mood
- Human studies · 45%45
- Size of the effect · 35%40
- Studies agree · 20%30
The studies used 6 to 9 times the daily serving in this bottle, and the pooled result was too weak to count.
In a 4-week study of 21 people with panic disorder, 12 g of inositol a day cut panic attacks more than a placebo 13. That is six times a 2,000 mg daily serving.
A review of 11 small studies on depression and anxiety found no clear effect overall 12. There was a trend toward help for women with severe PMS mood symptoms (PMDD).
Helps you sleep
- Human studies · 45%25
- Size of the effect · 35%35
- Studies agree · 20%30
One small study. Not enough to buy it for sleep.
We found one randomized study. In 60 pregnant women, 2,000 mg of myo-inositol a day for 10 weeks improved a standard sleep score by about 1.5 points on a 21-point scale 14.
That is one study, in pregnancy, with a modest change. Owners report both sides: “I sleep better at night” and “insomnia if I take the second dose too late in the day.”
That covers what inositol can do. Next is what owners ask most: will it upset my stomach or my cycle?
Studies: [14]
Will inositol upset my stomach or mess up my cycle?

Across 4 studies, side effects were reported by 7% of women on inositol and 53% on metformin 3. A safety review found that only the highest dose tested, 12 g a day, caused mild nausea, gas and diarrhea 15.
Owners report more than the studies do, mostly in week one: headaches, sleepiness, cramps or feeling wired. “For the first 5 days I got a migraine and fatigue... a week later side effects are gone.” Their advice: “Start the myo low and work up.”
A few owners felt hungrier or gained weight in the first weeks. One said the extra hunger “improved once I switched to taking it before bedtime.”
The first period after you start can be a shock. Owners write “Holy Cow. This period is intense!” and “I spotted the whole time between periods for 2 cycles.” Some get a late first period instead, and many quit here in a panic.
In owner reports, it usually settles by the second cycle. If bleeding is very heavy or lasts more than about a week, call your doctor. The 40:1 capsule we picked has a 30-day money-back guarantee if week one does not suit you.
If you take metformin or insulin, inositol can add to their effect on blood sugar. A few owners report shaky, low-sugar mornings. Talk to your doctor before you add it. Most of the rest comes down to how you take it, so that is next.
- Start with 1 capsule a day and build up over a week
- Take it with food, or in the middle of a meal
- Take the second dose with dinner, not at bedtime, if it keeps you awake
- Expect the first period to be different, and track it
- Stop and call your doctor if you get hives, a racing heart or very heavy bleeding
How much inositol should you take, and when?
Many of the strong studies used 2 g of myo-inositol twice a day, 4 g in total 1,4,8. The bottle we picked gives 2,000 mg of myo-inositol and 50 mg of D-chiro-inositol in 4 capsules. That is the 40:1 ratio, at half the 4 g used in many studies.
Take the label serving: 4 capsules a day, 2 with breakfast and 2 with dinner. Do not take more than the label says on your own. If you want the full 4 g from the studies, ask your doctor first.
More D-chiro is not stronger. In the small ratio study, raising the D-chiro share made results worse, not better 6. Your body keeps about 40 times more myo than D-chiro in the blood, and the 40:1 mix copies that.
The dose is the easy part. The hard part is finding a bottle that delivers it, so here are the traps.
1Days 1 and 2
1 capsule with breakfast.
2Days 3 and 4
1 capsule with breakfast and 1 with dinner.
3Days 5 and 6
2 capsules with breakfast and 1 with dinner.
4Day 7 on
2 capsules with breakfast and 2 with dinner, every day. Give it 3 months before you judge it.

Before you buy inositol anywhere: 6 warnings
You can buy inositol anywhere. Most bottles we looked at fail at least one of these.
1“2,000 mg” on the front, 4 capsules on the back
Most capsule bottles print 2,000 mg as the serving, then make it 4 capsules. Owners only find out later: “it's 120 count but 4 pills a day so it's only a 30 day supply.” Read the amount per capsule, not per serving. Dosera prints 500 mg of myo and 12.5 mg of D-chiro per capsule, and says plainly that 120 capsules last 30 days.2Myo only, D-chiro only, or the wrong ratio
The combined studies used myo and D-chiro at 40:1. In the ratio study, more D-chiro made results worse 6. Owners write “I took inositol (but it's not the 40/1 one)” when it did nothing. Dosera's capsule gives 2,000 mg of myo and 50 mg of D-chiro a day, the 40:1 ratio.3A few hundred mg in a gummy
4A “PCOS blend” with one number for ten ingredients
Herb blends list one total for the whole mix, so you cannot see how much inositol is inside. One owner asked the right question: “How do I know there is DCI in there and not just cornstarch?” Dosera lists both amounts on their own lines, with no proprietary blend.5Half-full bottles and empty capsules
Owners report “Bottle was half full” and “some of the capsules in the container were empty.” You cannot see that from a product photo. Dosera says every batch is lab-tested, and the 30-day money-back guarantee covers you if something is off.6Glowing reviews and refund windows that lapse
The FTC banned fake and bought reviews in 2024 18, but they still show up. Owners warn that “a lot of these products with slick marketing and cutesy branding are predatory.” Blood sugar buyers also wrote “Now that the trial period has lapsed I have no chance of getting my money back.” Dosera shows one price, $17.50 a bottle, and its 30-day guarantee needs nothing sent back.

How to choose an inositol supplement: 6 label checks
We read the labels of the main types of inositol product sold in the US. These 6 checks separate the ones that match the studies from the ones that do not.
1Myo + D-chiro at 40:1
The ratio used in the combined studies. Too much D-chiro made results worse in the ratio study 6.

Only two types pass every check, and one of them is a powder from a doctor's office. The powder means a scoop in water twice a day. The capsule means 4 capsules a day, with no scoop and nothing to taste. Some owners write “I will gladly take 4 pills over this powder.”
If capsules suit you, see the Dosera capsule that passed all six. Next: why we chose this maker over a famous name.
The maker we picked
Why we picked Dosera's inositol
We did not pick a famous name. We picked the bottle that matches the studies and passes all six checks above. That bottle is Dosera's 40:1 capsule.
The studied form and dose
Myo-inositol and D-chiro-inositol at 40:1: 2,000 mg of myo and 50 mg of D-chiro a day, in 4 capsules.
Every mg listed
Both amounts are printed per capsule and per serving. There is no proprietary blend.
Every batch lab-tested
Dosera says every batch is lab-tested, so the mg on the label should be the mg in the capsule.

Why we'd buy Dosera's inositol

Dosera Myo + D-Chiro Inositol 40:1
2,000 mg myo-inositol and 50 mg D-chiro-inositol (the 40:1 ratio) in 4 capsules a day
★ 4.8 from 1,912 reviews on the maker's page
We picked this bottle because it matches what the combined studies used: myo-inositol and D-chiro-inositol at 40:1, with both amounts printed. A serving is 2,000 mg of myo-inositol and 50 mg of D-chiro-inositol in 4 capsules. That works out to 500 mg of myo per capsule.
It is a capsule, so there is no scoop, no shaker and nothing to taste. Take 2 with breakfast and 2 with dinner. The maker says every batch is lab-tested, so the mg on the label should be the mg in the capsule.
That is our view. Here is what women who take inositol say, the good and the bad.
What it gets right
- Myo-inositol and D-chiro-inositol at the 40:1 ratio
- 2,000 mg myo + 50 mg D-chiro per serving, both printed
- No proprietary blend
- Lab-tested every batch
- Supports a regular cycle and healthy insulin function
- $17.50 a bottle, about 58 cents a day
- 30-day money-back guarantee, nothing to send back
What to know
- 4 capsules a day, which owners call “a lot”
- Takes 2 to 3 months to judge
- Like any inositol, it does nothing for some women
- The first period after you start can be heavy or late

Who should not take it
- Pregnant or breastfeeding, unless your OB says yes
- Taking metformin or insulin, unless your doctor knows first
- Told by your doctor to avoid D-chiro (some lean or adrenal PCOS)
- Anyone under 18
- Anyone with regular cycles and normal insulin who wants it for weight, sleep or mood

What do women who take inositol say?
These are real owner posts about inositol in general, not about the bottle we picked. We chose them to show the good, the bad and the first week.
“I can honestly say I brought my Period back and it feels great”
“Plain myo did nothing for me at all. But the 40:1 ratio... was life changing”
“My food noise is gone and I'm back to my regular period”
“Holy Cow. This period is intense!”
“for the first 5 days I got a migraine and fatigue after taking it... a week later side effects are gone”
“The only downside, if I had to choose something, would be the 4 pill dosage... I just take them in two gulps to prevent getting stuck”
“I still get chin hairs though so, sorry girlies - inositol didn't help that”
“I took it for around four almost five months and I didn't notice a single change”

What happens month by month when you start inositol?

1Week 1
Your body gets used to it
Some women get headaches or an upset stomach. Starting with 1 capsule, with food, helps. Some notice fewer sugar cravings.
2Weeks 2 to 6
The first bleed
Many owners get a period sooner than expected, often heavier or longer. Some get a late one instead. Track it rather than quitting.
3Months 2 to 3
Cycles shorten
Remember the woman on cycle day 45? An Amazon reviewer with long cycles wrote: “My cycle went from 58 days in June to 44 days. Now in September, it has gone down to 32 days between periods!” Studies on insulin and cycles ran about 12 weeks to 6 months.
4Month 6
Skin, hair and a decision
Acne and chin hair change slowly. If nothing has moved by now, it may not work for you. Talk to your doctor about the next step. Below is how we scored all of this.
How did we score each claim?
We listed the 8 benefits people search for most. For each one, we found the human studies and the reviews that pool them. We read each abstract on PubMed and wrote down the number of people, the dose, the length and the result.
We gave each claim three scores out of 100: human studies, size of the effect and whether studies agree. The index is those scores times the weights below. Then we read 665 owner quotes to see what people feel, and when.
- Only studies in people, never in cells or mice
- A good review counts more than one small study
- Doses far above a capsule bottle are scored down
- Owner quotes guide the side-effect and timing sections, not the scores
The weights
- Human studies45%
How many randomized studies in people tested this claim, how big they were, and whether a review pooled them.
- Size of the effect35%
How much changed in those studies. A big change in insulin or cycle length scores high. A small shift on a lab test scores low.
- Studies agree20%
Whether the studies point the same way. When a good review calls the evidence mixed or uncertain, this score drops.
What we did not score
We did not score “hormone balance,” because no study defines it. We did not score added folate or vitamin D, because the bottle we recommend does not carry them and you can buy them cheaply on their own.
That leaves the question owners ask at checkout: what should a month of inositol cost?
Why not the $10 bottle, or the $87 powder?

Per gram of myo, the $87 doctor's-office powder is a little cheaper than the capsule we picked. The powder runs $65 to $87 for a 90-day tub and gives 4 g of myo a day. That is about 18 to 24 cents a gram, against about 29 cents for our capsule.
Per month, the capsule costs less: $17.50 against about $22 to $29 for the powder. If your doctor wants you on the full 4 g and a scoop twice a day suits you, the powder is fair value. Owners call its price “ridiculous” at checkout, and some call it worth it once it works.
Owners judge inositol per month, not per bottle. A month of a 40:1 capsule at $13 to $28 feels normal, and $30 to $40 feels “spendy.” The common complaint: “it's 120 count but 4 pills a day so it's only a 30 day supply.”
The $10 bottles are usually plain myo, and owners report “some of the capsules in the container were empty.” They save you a few dollars and fail most of the checks above. Also avoid the blood sugar bundles sold through video ads. Buyers on Trustpilot wrote “I paid 400.00” and “I was charged twice before I even got my first order.”
Dosera's bottle is $17.50 a bottle, about 58 cents a day, at the low end of what owners call fair. It costs only a few dollars more than the plain myo bottles that fail the checks. Shipping in the US is free, and the 30-day money-back guarantee does not ask you to send anything back. Check today's price.
Price is one cost. The bigger one is waiting, so here is what the CDC says.
What happens if insulin resistance is left alone?

The CDC says more than half of women with PCOS develop type 2 diabetes by age 40 16. PCOS affects as many as 5 million US women. Insulin resistance is a big part of why.
It is not only a PCOS problem. More than 2 in 5 American adults have prediabetes 17. It often goes unnoticed until the A1c comes back high.
One of our analysts thinks we scored one claim too kindly. Here is her case.
Keep this in proportion. Inositol is a supplement, not a substitute for medicine, and it is not medicine for PCOS or diabetes. If you take metformin, insulin or birth control, keep your doctor in the loop before you add or stop anything. Ask for a fasting insulin and A1c test so you have real numbers. Anyone who promises a supplement will replace your prescription is selling you something. Walk away.

The analyst who disagreed
I think we scored acne and chin hair too kindly at 52. Chin hair is the slowest thing to change, and clients ask me about it more than anything else. The best study had no placebo group, and metformin beat it on hair. I would score it in the 40s and tell women to plan on laser or threading as well.

Questions people ask
Can I take inositol with metformin?
Many owners do, and some doctors recommend the pair. But both lower insulin, so together they can push blood sugar too low for some people. A few owners report shaky, low-sugar mornings. Talk to your doctor before you add inositol to metformin or insulin, and never stop a prescription on your own.
How long until I see a difference?
Most studies ran 12 weeks to 6 months. Owners often notice fewer cravings in the first 2 weeks. A first period may come in 1 to 6 weeks, and it may be heavy. Cycle length usually shifts by month 2 or 3. Acne and chin hair take about 6 months. If nothing has changed by then, talk to your doctor.
Should I take myo-inositol alone or with D-chiro?
Most combined studies used 40:1, and in the ratio study that mix did best 6. Raising the D-chiro share made results worse. Some doctors tell women with lean or adrenal PCOS to take myo alone. If yours has, follow that advice. Otherwise 40:1 is the safer bet.
Can I take inositol with berberine?
Owners ask this a lot. Both work on insulin, so together they add up, and the risk of low blood sugar goes up. Start one at a time, a few weeks apart, so you know which one is helping or upsetting your stomach. Read our berberine guide first, and check with your doctor if you take medicine for blood sugar.
Is it cheaper than the $60 to $87 brands?
Per month, yes. Some $60 to $87 inositol is a 90-day powder, about $22 to $29 a month. The 40:1 capsule we picked is $17.50 for a month. Per gram of myo, no. The powder gives 4 g a day, twice the label serving here, so each gram costs a little less. Other pricey bottles add herbs or a brand name. Myo-inositol itself is not rare. If a 40:1 bottle costs under $10, check the label and the capsule count.
Why not just buy a cheaper inositol on Amazon?
You can, but check four things first. The serving trick: “2,000 mg” on the front often means 4 capsules, so read the per-capsule amount. The missing dose: owners report “some of the capsules in the container were empty.” The wrong form: myo only, D-chiro only, or a ratio far from 40:1. Bought reviews: the FTC banned fake reviews in 2024, but they still show up 18. Dosera's bottle is $17.50 a bottle, passes all six checks and comes with a 30-day money-back guarantee with nothing to send back. See the bottle.
What are the side effects of inositol?
In studies, side effects were rare below 12 g a day 15. Owners mostly report week-one headaches, sleepiness, an upset stomach or a wired feeling, and a first period that is heavier or late. Start with 1 capsule and take it with food. Stop and call your doctor for hives, a racing heart or very heavy bleeding.
I take 2,000 mg. Should I go up to 4 g like the studies?
Owners ask this word for word. Stick to the label serving of 4 capsules a day. Many studies used 4 g of myo-inositol a day, twice the label serving. If you want the full study amount, ask your doctor first, and never take more on your own. Owners also say “start the myo low and work up,” because starting high brings headaches and nausea.
Can I take it on birth control, while trying to conceive, or when pregnant?
Owners take it while trying to conceive, often with their fertility doctor's approval. Ask your doctor before you combine it with birth control, letrozole or Clomid. If you get a positive test, ask your OB right away whether to continue. Doctors give different answers here.
Our verdict
If your periods are regular, your insulin is normal and you want inositol for weight, sleep or mood, skip it. The studies do not back those uses. If you are pregnant, ask your OB before you buy anything.
If you have PCOS, long cycles or insulin resistance, inositol is worth a 3-month try. It has the best human evidence of any PCOS supplement for lower insulin and more regular periods 1,4. It is far gentler on the stomach than metformin 3. It also does nothing for some women, so judge it in cycle days at month 3, not in week 2.
Here is our challenge. Find another 40:1 capsule at $17.50 a bottle or less with both amounts printed, every batch tested and a money-back guarantee with nothing to return. We looked and did not find one, which is why we picked Dosera. See the bottle we'd buy.
Inositol is worth 3 months if you have PCOS or insulin resistance. For weight, sleep or mood, the studies say skip it.
